Management Of Penil Zipper Entrapment Using Emla For Anaesthesia In Children

Abstract
Introduction: Zipper injuries of the penis are not uncommon and generally occur in uncircumcised boys. We determined the effectiveness of EMU (Etetic mixture of local anesthetic) for management of zipper foreskin entrapment.
Material and Method: Five boys with complaints of penile entrapment were taken into the study. In the first three boys, underwear were not being worn while the other two boys wore underwear. In the three boys that wore the underwear, the entrapment occurred while the boys doing up the zipper. However, in the two patients that wore the underwear the entrapment occurred while doing up in one and while undoing the zipper in the other.
Results: EMU at the same time served as a lubricant, was applied. Since the entrapment was seen to occur while doing or undoing the zip, one-sharp, upward or downward pull of the zipfastener solved the problem, accordingly.None of the m required circumcision and suffered any sequelae.
Condusion: Contrary to previous believes, release of entrapped foreskins were easily managed only by EMLA (not only for anesthesia but also for lubricating the foreskin) and pulling the zip fastener off, accordingly, without general anesthesia and ultimate circumcision.
